Early Self-Referral to Maternity Services at the L&D
Please note this form is for patients self-referring to the L&D
Once this form has been completed it will be sent to the Community Midwifery Services who will arrange your initial booking appointment with you, on the number you provide below.
If you are more than 9 weeks, please contact us as soon as you have submitted your referral form so we can process your referral and book you an appointment as soon as possible.
Please contact us if you have not heard from us within 15 working days, you can call 01582 497020, this number will get you through to maternity reception, Monday to Friday, 8am to 6pm.

Access to maternity, L&D
There is currently no pedestrian access via the main service road (off Lewsey Road) between 4pm and 1am until Saturday 3 May 2025. For those requiring pedestrian access to the maternity unit, please use the main entrance on Lewsey Road and follow the route through the hospital buildings and corridors. Alternatively, access can be gained from St Mary’s car park.
UPDATE: In addition, from 5 May to 16 May (previously 13 May), there will be no vehicle access (off Calnwood Road) to the main entrance of the maternity unit between 4pm and 1am Monday to Friday, and 8am to 4pm, Sunday and Bank Holiday Monday. Drop off and pick up will be in the St Mary’s car park only.
