Patient Information

How to Book an Appointment:

  1. A request for FSA or PVSA should be made on ICE by the requesting clinician.
  2. Once requested, an email should be sent to bhn-tr.lab-cellpath@nhs.net with the patient details to inform the department that a request on ICE has been made.
  3. On receipt of the email, the laboratory will print the request form from ICE and will allocate the next available appointment time for the patient.
  4. Patient will be sent details of the appointment, allowing a fortnight for delivery. A contact telephone number is provided to allow the patient to rearrange the appointment if the date/time is inconvenient. Patients will receive by post:
    • Date and time of their appointment
    • Patient instruction and information form. This forms includes a short Questionnaire to ascertain the patient’s sample is suitable for analysis. (If relevant in FSA, it is recommended to include the name of the patient’s partner on the request form for the correlation of results from both partners. If provided, this will be included on the report).
    • Pre-weighed and toxicity tested pot. NOTE: please do not give patients any other collection vial as this will lead to rejection of the samp

The patient must bring the sample to the laboratory on the appointed date stated on their paperwork. The sample should be delivered within 30-45 minutes of production. Please note that there are NO facilities on-site for sample production.

Failure to Attend:

If a patient fails to attend their allocated appointment, a letter will be sent to the requesting clinician. If the test is still required, please inform the laboratory by phone or email.
