Step 1: GP to seek prior funding approval from local CCG (where applicable) Tier 3 MUST have been recently completed before referring to Tier 4
Step 2: GP to refer to the Tier 3 service using the appropriate referral form and attaching the prior funding approval, where applicable.
Step 3: Once the approved referral is received, patients will be offered a place on the weight management programme. If the patient is seeking onward referral to Tier 4, the period of Tier 3 will depend on the individual CCG restrictions.
Step 4: End of Tier 3 Option 1: Referral to Tier 4 service for assessment for bariatric surgery (see step 5)
Option 2: Discharge back to GP for step-down referral to Tier 2 or community support for ongoing weight management
