BMI 35 – 40 with one or more of the following:

• Hypercholesterolaemia
• Type 2 Diabetes
• Metabolic Syndrome
• Obstructive Sleep Apnoea

OR

BMI 40+ without specified co-morbidity

PRIOR FUNDING APPROVAL

GP is required to seek prior funding approval to refer to Tier 3 weight management, unless BMI is above 50.

Tier 4 service is required to seek prior funding approval before accepting a referral to Tier 4.
